Eligibility Requirements
Seniors are eligible to be a participant in Trinity Health PACE Alexandria if, at the time of enrollment, they:
- Are at least 55 years of age.
- Are certified by the State to need nursing facility level of care.
- Live in the Trinity Health PACE Alexandria service area.
- Are able to live safely in the community with services provided by Trinity Health PACE Alexandria.
A PACE enrollee may be, but is not required to be, any or all of the following:
- Entitled to Part A,
- Enrolled under Part B,
- Eligible for Medicaid.
Trinity Health PACE Alexandria will not restrict enrollment based on Medicare or Medicaid eligibility.
Trinity Health PACE Alexandria services are typically free to seniors who qualify for Medicare and Medicaid or Medicaid only. There is a monthly charge for participants who do not qualify for Medicaid.
